AI failures have no villain

Look for who screwed up, and you'll find no one. No review was skipped, no vendor shipped something broken — every part did exactly what it was built to do, and harm emerged between them.

The problem isn't that someone failed. It's that everyone succeeded, and the system produced a loss anyway.
